Location: Fu Jen Catholic University Hospital

Bring the fee  (850 NTD during2021.10.13～2021.10.23)  and your personal ID (passport/ARC) when you go to the Fu Jen Catholic University Hospital for your health exam and fitness test at the time and date as per your online pre-registration.

Make or change your online pre-registration here

 

4.

Students who have not undergone health checkup: please download the student health information form from the Sanitation Division website and conduct a health exam on your own before 30thOctober 2021. The checkup medical report and the student health information form should be submitted to the Sanitation Division thereafter.
